Referral checklist: clinical question and previous imaging
Please identify the question the referral needs to answer and the referring clinician’s authorised contact details. Previous reports and original images support comparison; a written report alone may not provide everything needed for procedural planning.
- Relevant symptoms, examination findings, side and site of concern, and the reason for referral.
- Previous mammography, tomosynthesis, ultrasound or MRI reports and original image files, including DICOM where available.
- Relevant biopsy or cytology reports, earlier surgery or treatment details, and any marker or localisation records.
- Relevant medicines, allergies and other factors that may affect examination or procedure preparation.
- The clinician responsible for receiving results and coordinating the next step.
Breast imaging and intervention scope
Dr Pranjali Joshi’s training includes MD Radiodiagnosis at Dr Ram Manohar Lohia Institute of Medical Sciences, Lucknow, and PDCC Breast Imaging at AIIMS Rishikesh. Her work includes mammography, digital breast tomosynthesis, breast ultrasound and image-guided breast procedures.
Referral discussions may concern assessment of a breast finding, image-guided sampling, marker placement or localisation before surgery. The clinical question and available imaging guide planning; a requested procedure is confirmed after review.
Confirm the procedure and site
Dr Joshi leads Breast Imaging & Interventions at Advanced Breast Care at Lucknow Imaging Centre. Confirm the planned examination, location, preparation and timing with the practice before directing a patient to attend.
Her CV includes experience in stereotactic biopsy and vacuum-assisted breast biopsy. The appropriate technique, equipment and availability for a particular referral must be confirmed with the practice. The service pages describe procedures; they do not guarantee availability at every location or on every day.
Localisation planning with the surgical team
For localisation referrals, coordinate the target, laterality, planned operation and timing with the radiology and surgical teams. Include the imaging and pathology that identify the lesion, together with any information about an existing marker.
The teams should confirm which finding is to be localised and how the localisation information and any requested specimen assessment will be communicated. A referral enquiry does not confirm a procedure slot or surgical schedule.
Imaging and pathology communication
When sampling is planned, clarify the target and the clinical question for the pathology request. Ensure the referring team has access to the imaging report, procedure record and subsequent pathology report.
Imaging findings and pathology findings need to be considered together. Where the results do not adequately explain the imaging or clinical concern, the treating team may need further discussion and assessment. Agree who will communicate results to the patient and arrange follow-up.
Coordinate a referral
Contact the practice with your name, professional contact details and a brief description of the service you would like to discuss. The date, time, location and procedure are confirmed separately by the practice.
A shared plan for the imaging question
Discuss the referral with the practice and confirm the records and arrangements needed before the patient travels.
